Licensed Practical Nurse - LPN Visit Nurse Responsibilities:
As an LPN Visit Nurse, you'll play a vital role in providing comprehensive care to our clients, including:

• Conducting initial and ongoing assessments of clients' medical condition and vital signs.
• Developing and implementing individualized care plans in coordination with the healthcare team.
• Administering medications and treatments as prescribed by physicians.
• Monitoring clients' progress and reporting any changes in their medical condition.
• Providing education to clients and their families on disease management and home care techniques.
• Assisting with daily living activities and ensuring the safety and comfort of clients at all times.
• Documenting all patient care activities and maintaining accurate medical records.
• Communicating effectively with the healthcare team and other stakeholders to ensure the best possible patient outcomes.

Licensed Practical Nurse - LPN Visit Nurse Qualifications:
To excel in this role, you'll need:

• A current and valid Practical Nurse / LPN license in the state of Minnesota.
• Minimum of 1 year of experience in home healthcare or a related field.
• Strong clinical and critical thinking skills, with excellent communication and interpersonal abilities.
• Ability to work independently and as part of a collaborative team.
• Reliable transportation and willingness to travel to patient homes.
